4) Health guidelines for the event (for you to keep)
5) Dept of Revenue form (for you to report sales)
Download your form and the additional forms for food, fill them all out, sign where stated and return 4 pages with your check. It is required that you send all of the appropriate signed forms to us with the fire & health forms to secure your space.
