3) Health Department form (sign & return)
4) Health guidelines for the event (for you to keep)
5) Dept of Revenue form (for you to report sales)
Download forms, fill them all out, sign where stated and return 4 pages with your check. It is required that you send all of the appropriate signed forms to us with the fire & health forms to secure your site.
